FACIAL TREATMENT CONSENT AND RELEASE

For all Face Yoga Gym sign-ups, memberships, facial practices, and related activities.

Your purchase of, or enrollment in, any of our memberships or programs indicates your acknowledgment, agreement, and warranty of the following statements:

1. Acknowledgment of Treatments

I understand that beauty treatments, skincare practices, and massage techniques—including but not limited to gua sha, facial toning, body treatments, face yoga, face massage, facials, cosmetic products, masks, and facial oils—are offered as part of the services or programs.

I acknowledge that:

• Beauty procedures are not an exact science and no specific guarantees can be made regarding results.

• Individual results vary, and some clients may experience greater change or improvement than others.

• In most cases, consistent practice and/or multiple treatments are required to see noticeable results.

2. Risks and Hazards

I understand and agree to assume the potential risks and hazards associated with any treatment or practice, which may include but are not limited to:

• Unsatisfactory results

• Soreness or muscle tenderness

• Poor healing

• Discomfort or redness

• Blistering or skin irritation

• Allergic reactions

I understand that while reasonable precautions are taken, not all risks can be anticipated or eliminated in advance.

3. Variability of Results

I acknowledge that responses to treatments vary from person to person, and specific results are not guaranteed.

4. Release of Liability

In consideration for any products, tools, or treatments provided, I agree to unconditionally defend, indemnify, and hold harmless:

• The insured parties, additional insureds, officers, directors, and employees of the above

5. Confirmation

By signing up, purchasing, or participating in any membership, practice, or treatment, I confirm that I have read, understood, and agree to the terms of this Consent and Release.
